@Trung: I think you misunderstanding my question, i.e what you should have done but you have not done to achieve your career objective. For me, 3 things I should do and do more is:
- read every day for at least 30m (not like every other day like right now)

Here are 3 things that people do not do to achieve their top:
1- Accept challenge they are only 70% fit for and trust themselves to fullfill the 30% missing by themselves... you don't need to be 100% ready to take a top job.
2- Do things out of their comfort zone by themselves. If you can't get someone to do that mailing list, that website, that business plan by your chief accountant... go do it by yourself, get out of your comfort zone, 80% done is better than nothing done.
3- find and train that someone who will replace you so you can move on, you need to find/train your replacement.I made these mistakes myself.

1.Even though I think I am good in interaction but still I need to improve my human relations skills. I'm not that perfect so I can't be friendly with everyone. I understand that it is very hard to be friendly with the person I don't like. I really want to become friendly and kindness under everyone's eyes.
2. I didn't put enough time to enhance my reputation. I have to get involved with other professional organizations which allow me to grow my networks.
3. I didn't not maitain my habit to keep track of my clients at regular interval. When I got too busy I usually forget this task.
4. Sometimes I feel that I am behind the knowledge that I needed. I tend to relax, and forget to take responsiblity for my own profesional developement.Since you ask 3 things I give you 4 things. I believe that I have more things needed to developing and improving. I want to get better and better in each day.
